Five Nursing Homes Fined Following Lapses In Care

The state Department of Public Health (DPH) has fined five nursing homes for violations, most of which resulted in injuries to residents. The Springs at Watermark, East Hill, Southbury, was fined $1,300 after two residents were injured. In September 2014, a resident who required an ankle and foot orthotic for transfers and walking – suffered a skin wound due to improper use of the orthotic, according to the DPH. An investigation found that the resident’s care plan lacked documentation about the use of the orthotic, according to the citation. Also, in July 2015, a patient with various diagnoses, including anxiety and a history of falls, was hurt in a fall while being helped to the bathroom, according to the DPH.

Two Nursing Homes Cited After Resident Deaths

Seven nursing homes were recently fined more than $1,000 each by the state Department of Public Health, including a Bridgeport home and a Hamden facility that were each cited in connection with the death of a resident. The residents who died were at Bridgeport Manor and Arden House Rehabilitation & Nursing Center in Hamden. On Nov. 24, Bridgeport Manor was fined $1,020 in connection with the Oct. 6 death of a resident whose tracheostomy tube was dislodged.

Three Nursing Homes Fined

Three Connecticut nursing homes have been fined more than $1,000 each in connection with incidents that left three residents suffering broken bones. The Fairview Healthcare Center of Greenwich was fined $1,300 on Aug. 8, according to a citation released by the state Department of Public Health. The citation states that a resident with multiple sclerosis fell out of a lift in June because nurses’ aides used a sling that was too large. The resident was treated at a hospital for a head injury, bruised hip and a broken collarbone, the citation states.

Three Nursing Homes Face State Fines

Three nursing homes in southeastern Connecticut face state fines of more than $1,000 for lapses in care cited by the Department of Public Health. • Kindred Nursing and Rehabilitation Crossings East of New London was fined $1,090 by the DPH for two incidents in which staff members acted inappropriately with patients. In one case, a nurse physically restrained a resident who was behaving aggressively and engaged in an inappropriate verbal exchange, the DPH report says. In another, a nurse’s aide called a resident a derogatory name and slammed the door when she exited the resident’s room. The aide was terminated after the facility investigated the incident, the DPH report says.

State Raises Financial Penalties For Nursing Homes

Last summer, the state Department of Public Health fined a Danielson nursing home $580 after a resident whose feet were not properly secured to a wheelchair suffered a hip fracture. This January, a Waterbury nursing home was fined double that amount — $1,160 — after an incident in which a resident sustained a cut on the forehead during a fall, while being transferred to a toilet without proper precautions. Similarly, the Lutheran Home of Southbury paid a $615 state fine last spring, after a resident who was supposed to be served a “soft diet” was instead served a meal of ham and carrots and choked to death. Then more recently, in February, the state fined the Paradigm Healthcare Center of Norwalk double that amount — $1,230 — in connection with the death of a resident who choked on food that had been left on a tray at a nursing station. Why the differences in penalties?
